Insurance & Payment Options 

At Elevate Life Psychiatry, I am committed to making high-quality integrative psychiatric care accessible and affordable. Starting January 2026, I will begin accepting select health insurance plans — both in-network and in the process of credentialing — to support a broader range of patients.

Insurance Participation

In-Network Plans I am currently in-network with:

  • Aetna
  • Blue Cross Blue Shield (BCBS)
  • Cigna
  • Oscar Health
  • UnitedHealthcare (UHC)
  • Multiplan

Plans in Credentialing / Pending I am actively working toward in-network participation with the following carriers:

  • TriWest (In-Process)
  • Ambetter (In-Process)
  • Magnacare (In-Process)
  • Superior (In – Process)

Please note: Credentialing timelines vary by insurer; “In-Process” means we have submitted documentation and are working toward approved status.

What This Means for You

  • If your plan is in-network: I will bill your insurance directly for covered services.
  • If your plan is not yet in-network or out-of-network: You may still be eligible for reimbursement by your insurance company. I can provide a superbill (a detailed receipt) that you can submit to your carrier for potential reimbursement. I recommend checking with your insurance provider to understand your mental health benefits, deductible, copay, and reimbursement policies.

Cash-Pay & Superbills

I continue to offer cash-pay services for all patients. If your insurer does not yet credential our practice, you can still access care using superbill reimbursement options and tools that simplify claims submission.

Simplified Reimbursement with Thrizer

If I am not in network with your insurance plan, I’ve partnered with Thrizer, a free tool that makes it easier for you to get reimbursed by your insurance. If you have a PPO plan with out-of-network benefits, Thrizer takes the hard work out of the process.

Instead of paying the full session fee upfront and filing claims yourself, Thrizer allows you to just pay your estimated portion—and they handle the rest. No paperwork, no stress.

Forms

  • Forms will be filled out only for established patients.
  • Forms that may be provided, at the clinician’s discretion, could be 504 letters, ESA letters, etc
  • I do not offer long term disability certification.
  • Short-term disability may be considered for established patients who are in a psychiatric day program that requires time off work. This will be at the clinician’s discretion.
  • Any form requiring evaluation and more than 10 minutes of the clinician’s time will need adequate time to be filled out with the patient during the clinic visit. As such the appointment requirement is at least a 40-minute appointment.

Labs & Additional Services

Lab testing may be recommended and may be covered by insurance depending on your plan. In some cases, separate billing may occur through external lab providers.

No Surprises Act

In compliance with the “No Surprises Act” starting on 1/1/2022 “Good Faith Estimates” are available upon request and will be provided to all patients within 3 business days.
